How to apply for vocational rehabilitation
You can apply for vocational rehabilitation using a PDF form. The application must be accompanied by a medical statement.
If you are applying for vocational rehabilitation for young persons, read the instructions for applying here.
- First see your doctor, who will assess your need for rehabilitation. If necessary, your doctor will then recommend rehabilitation, and write a statement on it (medical statement B).
- Fill in the application form for vocational rehabilitation KU 101e (PDF).
- Save the blank form to your device. Do not fill in the application directly in the browser, otherwise some of the information may not be saved.
- Open the form with Adobe Reader and fill it in.
- Among the alternatives listed on the form, select the type of rehabilitation for which you are applying. If you do not know what type of rehabilitation would be right for you, you can call our customer service and book an appointment for an interview where we will investigate your rehabilitation needs.
- If your application concerns assistive devices, fill in the form KU103 (PDF, in Finnish).
- Save the completed form to your device. Note that the form must be in PDF format.
Send the application using the OmaKela e-service.
- Log in to OmaKela (available in Finnish and Swedish).
- Select the application form on your device and press Lähetä (Send).
- When you send your application online, you do not need to sign it.
Next, submit the supporting documents in OmaKela. If they are paper documents, you can photograph or scan them.
Submit the medical statement B if your doctor has not already submitted it to Kela.
Also submit:
- the vocational statement form for Kiila rehabilitation KU 200 (PDF, in Finnish) if you are applying for Kiila rehabilitation. The form must be completed together with your supervisor.
- copies of school diplomas and job certificates if you are applying for education or training.
- the appendix form for the start-up grant for self-employment KU 105 (PDF, in Finnish) as well as a detailed account of the types of tools you use and of the profitability of your self-employment if you are applying for a start-up grant for self-employment.
After Kela has received your application, you may be invited to an interview. The interview is used to find out more about your rehabilitation needs.
- Once your application has been reviewed, you will receive a written decision by post. You can also view the decision in the OmaKela e-service.
The decision will be communicated to the rehabilitation provider, who will reserve a place for you in the rehabilitation and accept it as a voucher covering the cost of the rehabilitation. The decision will also be sent to the doctor who recommended the rehabilitation.
Alternatively, you can send your application by post.
- Sign the application and mail it along with the supporting documents to Kela, PL 10, 00056 KELA.
When to apply
File your application with Kela before the start of the rehabilitation. Please note that if the rehabilitation measure has already started, Kela can only cover it from the beginning of the month of application. In exceptional cases, it is possible to move the application deadline backwards by six months.
